What is NYS PCMH?
The National Committee for Quality Assurance and New York State have collaborated to create the New York State Patient Centered Medical Home (NYS PCMH) program. Using evidence-based quality metrics, the program is a transformation model that provides primary care providers with a set of guidelines for improving the quality and delivery of care while meeting patients’ multiple needs.
NYS PCMH is focused on six key areas: Team-Based Care, Knowing Patients, Access and Continuity, Care Management, Care Coordination, and Quality Improvement. HCG Systems provides practices with the tools they need to incorporate these six concepts into their routine workflow, measure and track their performance, and report their results to the appropriate agencies to participate in value-based payment programs.
